Leaving Hospital
If you are in hospital and worried about your housing situation when you leave, ask hospital staff to refer you to the hospital discharge team as soon as possible. This might be because you have no settled accommodation or if your accommodation is unsuitable.
The discharge team should refer you to the housing needs team at the council through a Duty to Refer
We will have a duty to assess your housing situation and identify what your housing and support needs are, and to work with you to try to prevent your homelessness or support you to find you somewhere to live.

- Small home adaptations such as grab rails in the bathroom and at doorways in the home.
